The mission of HomeBridge is to offer supportive services in one's home environment to promote choice and independence with a network of providers, while remaining consistent with the ministry of Westminster Towers.

HomeBridge is focused on supporting an individual's independent functional capacity, while offering the necessary provider partnerships for the client to remain in their home environment for as long as possible. It is the intent to assist and support, but not replace family members in carrying out personal responsibilities for their loved one's well being.

To develop a menu of services forged from collaborative relationships of community organizations, churches, Home Health and Hospice programs, physicians, local businesses and developments in technologies aimed to increase safety and enrich the lives of the individuals served, in their choice of environment.

Our focus is on providing support and wellness to seniors! We offer the services of a professional nurse 24/7 that can assist in developing an individualized plan of support, at one's address of choice! If the needs are beyond our scope of services, we will suggest options for an appropriate provider. Afterall, it's about:
